Cetirizine; Pseudoephedrine (Zyrtec-D) Indications and Dosage

For the treatment of seasonal or perennial allergic rhinitis and accompanying nasal congestion:
Oral dosage:
Adults, the elderly, and adolescents >= 12 years: 1 tablet (cetirizine 5 mg and pseudoephedrine 120 mg extended-release) PO up to twice a day as needed, with doses given 12 hours apart. Elderly patients are more likely to have reduced renal function and adverse reactions to cetirizine; pseudoephedrine.
Children < 12 years: Safe and effective use of this fixed-dose combination product has not been established.

Maximum Dosage Limits:

  • Adults: 10 mg/day PO cetirizine and 240 mg/day PO pseudoephedrine.
  • Elderly: 10 mg/day PO cetirizine and 240 mg/day PO pseudoephedrine.
  • Adolescents >= 12 years: 10 mg/day PO cetirizine and 240 mg/day PO pseudoephedrine.
  • Children < 12 years: Not recommended.

    Patients with hepatic impairment:
    No more than 1 tablet (cetirizine 5 mg and pseudoephedrine 120 mg extended-release) PO once daily for hepatically-impaired adults and adolescents >= 12 years.

    Patients with renal impairment:
    CrCl >= 32 ml/min: No dosage adjustment needed.
    CrCl <= 31 ml/min: No more than 1 tablet (cetirizine 5 mg and pseudoephedrine 120 mg extended-release) PO once daily for renally-impaired adults and adolescents >= 12 years.

    Intermittent hemodialysis:
    Follow dosage recommendations as for CrCl <= 31 ml/min; cetirizine is not removed by hemodialysis. The effect of hemodialysis on the removal of extended-release pseudoephedrine is unknown; however, 38% of immediate-release pseudoephedrine is removed by hemodialysis. Supplemental dosing is not needed.
