Tamiflu (Oseltamivir) Indications...Dosage
The following viruses are generally considered susceptible to Tamiflu (Oseltamivir) in vitro: influenza A virus; influenza B virus.
For the treatment of uncomplicated acute influenza A virus infection or influenza B virus infection in patients who have been symptomatic for no more than 2 days (i.e., <= 40 hours after onset of influenza symptoms):
Oral dosage:
Adults and adolescents >= 13 years: 75 mg PO twice daily for 5 days. No increase in efficacy was noted with dosages > 150 mg/day PO.
Children (weight > 40 kg): 75 mg (6.2 ml) PO twice daily for 5 days.
Children (weight > 24-40 kg): 60 mg (5 ml) PO twice daily for 5 days.
Children (weight 16-23 kg): 45 mg (3.8 ml) PO twice daily for 5 days.
Children (weight <= 15 kg): 30 mg (2.5 ml) PO twice daily for 5 days.
Infants < 1 year of age: Not indicated.
For influenza prophylaxis for infections due to influenza A or influenza B virus following close contact with an infected individual or during a community outbreak of influenza:
Oral dosage:
Adults and adolescents >= 13 years: 75 mg PO once daily for at least 7 days following close contact and for up to 6 weeks during a community outbreak. The duration of protection lasts for as long as dosing is continued. In a large study, subjects were randomized to placebo, Tamiflu (Oseltamivir) 75 mg PO twice daily, or Tamiflu (Oseltamivir) 75 mg PO once daily for 6 weeks at the beginning of an influenza outbreak. The proportion of subjects with laboratory-confirmed influenza-like illness was significantly lower in both Tamiflu (Oseltamivir) treatment groups than placebo. There was no significant difference between the Tamiflu (Oseltamivir) treatment groups. A second study, used 75 mg PO once daily for 7 days and reported Tamiflu (Oseltamivir) was 92% effective in reducing the incidence of influenza in adults and adolescents who were in contact with an infected household member (manufacturer data on file). As an alternative to influenza vaccination in HIV-positive patients, the CDC suggests Tamiflu (Oseltamivir) 75 mg PO once daily.
Elderly: 75 mg PO once daily for at least 7 days following close contact and for up to 6 weeks during a community outbreak. Tamiflu (Oseltamivir) treatment for 42 days has been efficacious in preventing influenza in 92% of those in residential nursing home settings during a flu outbreak; note that 80% of the residents had previously received influenza vaccination (manufacturer data on file).
Adolescents and children < 13 years: Safe and effective use is not established.
Infants < 1 year of age: Not indicated.
Maximum Dosage Limits:
- Adults: 75 mg PO twice daily for 5 days.
- Elderly: 75 mg PO twice daily for 5 days.
- Adolescents >= 13 years: 75 mg PO twice daily for 5 days.
- Children >= 1 year: Maximum dosage is dependent on weight; individualize.
- Infants < 1 year: Not indicated.
Patients with hepatic impairment:
The safety and efficacy of Tamiflu (Oseltamivir) in patients with hepatic impairment have not been established.
Patients with renal impairment:
CrCl 10-30 ml/min: decrease adult dose to Tamiflu (Oseltamivir) 75 mg PO once daily for 5 days for influenza treatment; for influenza prophylaxis, decrease dose to 75 mg PO every other day.
CrCl < 10 ml/min: Tamiflu (Oseltamivir) has not been studied in this population; therefore, administer with caution.
Intermittent hemodialysis:
As for CrCl < 10 ml/min, Tamiflu (Oseltamivir) has not been studied in this population. No dosage adjustment receommendations are available for treatment or prophylaxis.
