Spironolactone Administration

NOTE: Since the onset of action of spironolactone is gradual, partially explained by the long half-life of canrenone (active metabolite), several days should elapse between dosage adjustments.

NOTE: When spironolactone is added to another diuretic, it is recommended that the other drug be reduced by one half and then adjusted to individual requirements and tolerance. Geriatric patients may be more sensitive to the effects of normal adult doses.

Oral Administration


  • Administer with food.
  • Extemporaneous formula for oral suspension : An oral suspension with a final concentration of 1 mg/ml can be compounded using spironolactone tablets: Pulverize ten 25 mg tablets; add a small amount of purified water and allow to dissolve for 5 minutes. Add 50 ml of 1.5% carboxymethylcellulose, and 100 ml of simple syrup NF and mix. Use a sufficient quantity of purified water to a total volume of 250 ml. Shake well. The resulting suspension (1 mg/ml of spironolactone) is stable for 3 months at room temperature or refrigerated.
