Phentermine Indications and Dosage
For short-term treatment of obesity, used as an adjunct to behavior modifications including dietary modification and appropriate exercise [NOTE: Phentermine is indicated for obese patients with a BMI >= 30 kg/m2 or a BMI >= 27 kg/m2 in the presence of other risk factors (e.g., hypertension, diabetes mellitus, or dyslipidemia). Oral dosage (phentermine hydrochloride 8 mg dose forms):
Adults, the elderly, and adolescents: 8 mg PO three times per day, 30 minutes before meals or at least 1-2 hours after meals. The elderly may require lower dosage.
Oral dosage (phentermine hydrochloride 15 mg or higher dose forms):
Adults, the elderly and adolescents: 15-37.5 mg PO once per day as a single dose every morning before breakfast or approximately 2 hours after breakfast. Dose strengths of 15 mg or higher of phentermine hydrochloride are to be dosed once per day, even though they are not formulated as ‘sustained-release’ products. The elderly may require lower dosage.
Oral dosage (phentermine resin complex):
Adults, the elderly and adolescents: 15-37.5 mg PO once per day as a single dose before breakfast or at least 10-14 hours before bedtime. Phentermine resin complex is only administered once-daily. The elderly may require lower dosage.
Maximum dosage limits:
- Adults: 37.5 mg/day PO.
- Elderly: 37.5 mg/day PO. May require reduced dosage compared to younger adults.
- Adolescents > 16 years: 37.5 mg/day PO.
- Adolescents <= 16 years: Safe and effective use has not been established.
- Children: Safe and effective use has not been established.
Patients with hepatic impairment:
The effects of hepatic impairment on the pharmacokinetics and clinical activity of phentermine metabolites is unknown; use with caution in patients with hepatic impairment.
Patients with renal impairment:
Specific guidelines for dosage adjustments in renal impairment are not available; however, phentermine is substantially eliminated by the kidney. Dose adjustment may be needed in patients with reduced renal function; consider monitoring renal function at baseline and throughout therapy, if indicated.
