Esomeprazole (Nexium) Administration
Oral Administration
- Administer on an empty stomach, one hour before meals.
- May be taken concurrently with antacids.
- Esomeprazole capsules contain enteric-coated, delayed-release pellets. These pellets should not be chewed or crushed.
- Esomeprazole capsules can be opened and the enteric-coated pellets sprinkled on cold applesauce or yogurt, given with fruit juices, or swallowed immediately with water. Once sprinkled on applesauce or yogurt, the pellets should be only swallowed with the soft food and not chewed. Do not store the pellet/applesauce or pellet/yogurt mixture for future use.
- For patients who have a nasogastric (NG) tube in place, the capsules can be opened and the granules emptied into a 60 ml syringe and mixed with 50 ml of water. Replace the plunger and shake the syringe vigorously for 15 seconds. Hold the syringe with the tip up and check for granules remaining in the tip. Attach the syringe to a nasogastric tube and deliver the contents of the syringe through the nasogastric tube into the stomach. After administering the granules, the nasogastric tube should be flushed with additional water. Do not administer the pellets if they have dissolved or disintegrated. Administer the contents of the syringe immediately; do not store.
Intravenous Administration
- For intravenous administration only.
- Visually inspect parenteral products for particulate matter and discoloration prior to administration whenever solution and container permit.
- Storage of unopened vials: Store at controlled room temperature, 15 - 30 degrees C (59 - 86 degrees F). Protect from light. Store in carton until time of use.
Intermittent IV injection:
- Dilute dosage, either 20 or 40 mg, to a total of 5 ml with 0.9% Sodium Chloride for Injection (NS).
- Inject 5 ml of reconstituted solution over no less than 3 minutes.
- Do NOT administer concomitantly with any other medications through the same intravenous site and or tubing.
- The IV line should always be flushed with either NS, LR, or D5W both prior to and after administration of esomeprazole.
- Storage of reconsituted injection: Reconstituted solution should be stored at room temperature up to 30 degrees C (86 degrees F); administer within 12 hours of reconstitution.
Intermittent intravenous (IV) infusion:
- Reconstitute the contents of one vial, either 20 or 40 mg, with 5 ml of NS, D5W, or LR.
- Dilute the resulting solution in 50 ml of NS, D5W, or LR.
- Infuse over 10 - 30 minutes.
- Do NOT administer concomitantly with any other medications through the same intravenous site and or tubing.
- The IV line should always be flushed with either NS, LR, or D5W both prior to and after administration of esomeprazole.
- Storage: Reconstituted infusions should be stored at room temperature up to 30 degrees C (86 degrees F). Solutions diluted with NS or LR are stable for up to 12 hours; solutions diluted with D5W are stable for up to 6 hours.
