Cefoxitin Administration

Parenteral Administration

  • Administer intravenously or intramuscularly.
  • Do not use diluents containing benzyl alcohol for preparing doses for pediatric patients. Benzyl alcohol has been associated with toxicity in neonates and may also cause toxicity in some smaller pediatric patients.
  • Visually inspect parenteral products for particulate matter and discoloration prior to administration whenever solution and container permit.

    Intramuscular injection:

  • Vials: reconstitute each gram with 2 ml of sterile water for injection. Alternatively, for IM use only, each gram may be reconstituted with 2 ml of 0.5% lidocaine HCl (without epinephrine) to minimize IM discomfort.
  • Inject appropriate dose deeply into a large muscle (i.e., upper outer quadrant of the gluteus maximus or lateral part of the thigh). Aspirate to avoid injection into a blood vessel.

    Direct Intermittent IV injection:


    • Use reconstituted powder in vials only.
    • Vials: Reconstitute 1 g with at least 10 ml and 2 grams with 10 or 20 ml of Sterile water for injection, Bacteriostatic water for injection, Sodium chloride 0.9%, or Dextrose 5%. These solutions maintain satisfactory potency for 6 hours at room temperature or for one week under refrigeration (below 5 degrees C).
    • Inject appropriate dose directly into a vein over 3 - 5 minutes or slowly into the tubing of a freely-flowing compatible IV solution.

    Intermittent or continuous IV infusion:


    • Vials: Reconstitute 1 g with at least 10 ml and 2 grams with 10 or 20 ml of Sterile water for injection, Bacteriostatic water for injection, Sodium chloride 0.9%, or Dextrose 5%. These primary solutions maintain satisfactory potency for 6 hours at room temperature or for one week under refrigeration (below 5 degrees C). These solutions may be further diluted with 50 to 1000 ml of a compatible IV solution such as Sodium chloride 0.9%, Dextrose 5% or 10%, Dextrose 5% and Sodium chloride 0.9%, Dextrose 5% with 0.2% or 0.45% saline, Dextrose 5% in Lactated Ringer’s, Lactated Ringer’s, Sodium bicarbonate 5%, Sodium lactate M/6, or Mannitol 5% or 10%. After dilution, the solution maintains satisfactory potency for an additional 18 hours at room temperature or an additional 48 hours under refrigeration.
    • Infusion bottles: reconstitute bottles containing 1 or 2 g with 50 - 100 ml of Sodium chloride 0.9%, Dextrose 5% in water, or Dextrose 10% in water. The solution maintains satisfactory potency for 24 hours at room temperature or for 1 week under refrigeration (below 5 degrees C).
    • Pharmacy bulk packages: reconstitute 10 grams with 43 or 93 ml of a compatible solution (e.g., Sterile water for injection, Bacteriostatic water for injection, Sodium chloride 0.9%, or Dextrose 5% in water) to give concentrations of 200 mg/ml or 100 mg/ml, respectively. FURTHER DILUTION IS REQUIRED. Dilute appropriate dose in 50 to 1000 ml of a compatible IV solution. After dilution, the solution maintains satisfactory potency for an additional 18 hours at room temperature or an additional 48 hours under refrigeration.
    • ADD-Vantage® vials: for IV infusion only. Reconstitute only with Sodium chloride 0.9% or Dextrose 5% in water in the appropriate 50 or 100 ml flexible diluent container. The solution maintains satisfactory potency for 24 hours at room temperature.
    • Premixed solutions (e.g., Mefoxin® Galaxy containers): Follow manufacturers directions for preparing for administration.
    • Infuse appropriate dose IV over 20 - 60 minutes.
