Allegra-D Indications and Dosage

For the treatment of allergic rhinitis:
Oral dosage (Allegra-D® extended-release tablets; 60 mg fexofenadine and 120 mg pseudoephedrine):

Adults and adolescents: One tablet PO twice daily.

Elderly: Initially, 1 tablet PO once daily; may increase to 1 tablet PO twice daily if tolerated. Elderly patients are more likely to have adverse reactions to sympathetic amines and reduced renal function.

Children: Safe and effective use has not been established.

Oral dosage (Allegra-D® 24-Hour extended-release tablets; 180 mg fexofenadine and 240 mg pseudoephedrine):

Adults: 1 tablet PO once daily.

Elderly: 1 tablet PO once daily. Elderly patients are more likely to have adverse reactions to sympathetic amines and reduced renal function.

Adolescents: 1 tablet PO once daily.

Children < 12 years: Safe and effective use has not been established.

Maximum Dosage Limits:


  • Adults: 180 mg/day PO fexofenadine; 240 mg/day PO pseudoephedrine.
  • Elderly: 180 mg/day PO fexofenadine; 240 mg/day PO pseudoephedrine.
  • Adolescents: 180 mg/day PO fexofenadine; 240 mg/day PO pseudoephedrine.
  • Children < 12 years: Safe and effective use has not been established.

Patients with hepatic impairment:
No dosage adjustment is recommended. The pharmacokinetics of fexofenadine are not substantially different in patients with hepatic disease. The effect of liver disease on pseudoephedrine pharmacokinetics is unknown.

Patients with renal impairment:
CrCl < 80 ml/min: Tablets containing 60 mg fexofenadine and 120 mg pseudoephedrine should not be administered more frequently than once per day; tablets containing 180 mg fexofenadine and 240 mg pseudoephedrine should not be administered to patients with renal insufficiency.

Intermittent hemodialysis:
See dosage adjustment for patients with renal impairment. The effect of hemodialysis on the removal of fexofenadine is unknown. Hemodialysis did not effectively remove fexofenadine from blood (up to 1.7% removed) following terfenadine oral administration. Hemodialysis minimally removes pseudoephedrine from the blood.
